Privacy at this office

Short version: the callback form goes to Rose Chiropractic and nowhere else, and we do not sell or share what you tell us.

What the website collects

The callback form on this site asks for your name, a phone number, the kind of visit you are after, a preferred time of day and an optional note about your complaint. That is all it asks for, and all of it is optional except the name and phone number we need in order to call you back.

The site does not run advertising trackers, does not embed social media pixels and does not set marketing cookies. Standard web-server logs record the usual technical details of a request, such as the page fetched and an abbreviated network address, and are used only to keep the site running.

How we use it

A callback request is used for exactly one purpose: to return your call and, if you decide to come in, to open an appointment. We may keep the note you wrote so the doctor can read it before you arrive.

We do not add website enquiries to a mailing list. If you would like to hear from us about anything else, you have to ask.

Patient records

Once you become a patient, your clinical record is handled under the federal and New York State rules that apply to healthcare providers, including HIPAA. Those records are kept separately from website enquiries, are only accessible to the doctor and the office, and are released to a third party — an insurer, another clinician, an attorney — only with your written authorisation or where the law requires it.

You may ask to see your record, request a copy, or ask for a correction. The office will explain the process and any copying fee before you commit to anything.

Who else sees your data

The website is served by a commercial hosting provider that necessarily processes traffic to the site. Aside from that, no analytics company, advertiser or data broker receives information from this website.

For billing, an insurer or claims administrator receives the information required to process your claim, and nothing more.

How long we keep things

Unanswered or declined callback requests are deleted once they are no longer needed — typically within a few months. Clinical records are retained for the period required by New York State for chiropractic practices.

Children

We treat children, but the website is written for adults. A parent or guardian should submit any request on a child's behalf.
